Pursuant to Sections 59 and 60 of the Health Act 2007, an Interim Order was made by Carlow District Court, Carlow, Co Carlow on 21 July 2011 cancelling the registration of Avondale Nursing Home, West Street, Callan, Co Kilkenny as a designated centre under Part 8 of the Health Act 2007, with effect from 19:00hrs on 21 July 2011. In accordance with Section 64 of the Health Act 2007 the HSE was directed to take charge of Avondale Nursing Home from such time and to make alternative arrangements for the residents of Avondale Nursing Home as soon as practicable.

This guidance document aims to set out criteria to quality assure clinical guidelines which will support continuous improvement in the quality and safety of healthcare in Ireland.

In April 2009, the Health Information and Quality Authority published its investigation report into Mid-Western Regional Hospital (MWRH) Ennis.

The Authority undertook the investigation following serious concerns raised by family members following the deaths of two patients who received treatment at the hospital and further concerns by families about the quality of care received across a variety of services provided.

These reports highlight a number of immediate patient safety concerns which emerged during the course of these investigations.

Pursuant to Sections 59 and 64 of the Health Act 2007, an Order was made on consent by the District Court, Drogheda, Co Louth on 04 July 2011 cancelling the registration of Creevelea House Nursing Home, as a designated centre under Part 8 of the Health Act 2007, with effect from 1.00pm on 04 July 2011.

In accordance with Section 64 of the Health Act 2007, the Health Service Executive was directed to take charge of Creevelea House Nursing Home from such time and to make alternative arrangements for the residents of Creevelea House Nursing Home as soon as practicable.

The Health Information and Quality Authority has had ongoing concerns in relation to the quality and safety of the care provided to patients requiring acute admission to the Adelaide and Meath Hospital, incorporating the National Children’s Hospital (the Hospital), and who receive initial care in the Hospital’s Emergency Department, and has previously sought assurances in relation to how the Board and Executive of the Hospital are governing and managing these risks.
